Mouth Corner Botox

Mouth Corner Botox places a small dose of botulinum toxin into the small muscle that pulls the mouth corners downward, reducing that pulling force. With the downward pull weakened, the lifting force acts relatively more strongly — and the set of your mouth at rest looks less droopy.

▲ An anatomical drawing with the skin below the mouth folded back — the triangular muscle rising at an angle from the lower edge of the jawbone toward the mouth corner is the depressor anguli oris. True to its name, its job is pulling the mouth corner down

Around the mouth, muscles that lift the corners and muscles that lower them sit side by side. With age, or a habit of tensing the mouth, the lowering side gets used more often and more strongly. Botulinum toxin reduces the signal telling that muscle to engage — lowering only the downward pull.

When to Consider It

  • Your mouth corners sit turned down even with no expression
  • People say you look upset or sullen
  • Your mouth looks noticeably droopy in photos
  • The two corners sit at different heights
  • You want the impression refined while your lip line stays untouched

If the droopy look comes from hollowing beside the mouth corners casting a shadow, muscle isn't the cause — that calls for refilling the lost volume, and sometimes both are at play. We decide which it is together, based on your assessment.

A Very Small Muscle — the Amount Decides the Result

This muscle is among the smaller ones treated on the face, and the other muscles that move your lips sit right beside it. Even slightly too much, or a placement just slightly off, can affect your smile or your speech.

So this treatment comes down to one thing: a small amount, in exactly the right spot. Putting in more doesn't lift the corners further — it pushes the mouth toward looking unnatural instead.

How It's Done

1. Assessment A 3D MetaVu scan captures your facial contours and left–right balance, confirming how much the two corners differ in height.

2. Expression check We alternate between your mouth tensed and fully relaxed, confirming where the corners settle at rest.

3. Cause check We sort out whether the droop comes from the muscle or from hollowing beside the mouth.

4. Numbing A cold roller dulls the sensation before we start.

5. Injection A very fine needle places only a small dose, with the placement kept narrow so it doesn't spread to the neighboring muscles that move your lips.

6. Aftercare guidance We go over what to follow for the day, and how to check your expressions at 2 weeks.

Results and Recovery

Item Details
Where it acts The muscle that pulls the mouth corners down — below the fat layer, above the jawbone
What changes The force pulling the corners downward — and with it, the set of your mouth at rest
What doesn't change Your lips' size and shape · shadows cast by hollowing beside the mouth
When results show From days 3–7, clearest at week 2 — the signal doesn't cut off at once; it tapers over several days
How long it lasts 3–6 months
Repeat cycle 3–4 months — spaced by how much of the pulling force has returned
Pain Light, shallow mosquito-bite pricks
Pain control A cold roller dulls the sensation before we start
Downtime The marks at the injection points settle within 1–2 hours; you can wash your face and wear makeup after 2–3 hours · bruises, if any, fade within 3–7 days
For the day Don't lie down for 4–6 hours · skip saunas, hard exercise, and massaging the treated area — all to keep the product from spreading beyond where it was placed

The change is not a large one. This isn't a treatment that visibly lifts the mouth corners — the force that dragged them down eases, and the droopy impression softens. We tell you this first, before starting.

Expected Reactions and Possible Side Effects

Common, temporary reactions — part of the normal response:

  • Marks at the injection points, settling within 1–2 hours
  • Bruises, if any, fading within 3–7 days
  • Possible early awkwardness in your smile or speech — which is why we start with a minimal dose

Anything beyond this is uncommon. If a reaction lasts longer than expected or worsens, contact us and we'll check it with you.

What We Check Beforehand

If any of these apply to you, let us know during your consultation. We postpone or adjust the timing.

  • Pregnancy or breastfeeding
  • Conditions affecting the point where nerves meet muscles, such as myasthenia gravis — the very site where botulinum toxin acts, so symptoms could worsen
  • Medications that affect nerve–muscle signaling (including some antibiotics)
  • Work that leans heavily on speech or mouth movement — early awkwardness is possible, so timing is adjusted
  • Hollowing beside the mouth — the cause may not be the muscle, so we check first
  • Inflammation or wounds at the injection site
  • Aspirin or blood thinners (anticoagulants) — bruising can be heavier, so tell us in advance and we'll plan for it
  • Botulinum toxin received elsewhere recently — tell us when, with which product, and how much, and we'll recalculate the total amount given in one session and the interval to your next treatment

How PECHE Clinic Does It

We start with a small dose. This isn't a spot that improves with more. With the left–right balance confirmed in your scan, your actual expressions, and standards built from over 90,000 treatments, we start from the minimum — then check at 2 weeks and top up if needed.

We offer four botulinum toxin products. Botulinum toxin is a substance produced by bacteria. The bacteria send it out wrapped in other proteins to keep it intact — but only the core inside produces the effect; the wrapping plays no part in it. The core is identical across all four products; what differs is that some products keep the wrapping, and some strip it away to leave only the core.

Product Made by Protein wrapping Albumin (storage aid)
Newlux Medytox, Korea Kept Used
Coretox Medytox, Korea Stripped away Not used
Xeomin Merz, Germany Stripped away Used
Allergan Botox AbbVie, USA Kept Used

What keeping or stripping the wrapping means. The wrapping protects the core — but it also means foreign proteins unrelated to the effect enter your body along with it. Repeated over the years, your body can come to recognize and respond to those proteins — and then the same dose may not work as well as it once did. Stripped products leave less room for that. For a one-off treatment the difference is negligible; it matters when you plan to continue over several years.

Albumin is a common protein naturally present in human blood. It keeps the core from degrading while the drug is stored as a dried powder — and Coretox does without it too. The mouth corner takes a very small amount, so how narrowly it stays put is what we look at first — spread to the neighboring muscles, and the mouth turns awkward.

We open the genuine product in front of you — and you can watch it being dissolved through the glass wall, too. Botulinum toxin doesn't keep long once dissolved, so it's stored as a powder and dissolved in saline just before treatment. The more diluted it is, the larger its volume — and the wider it spreads. In a treatment that uses a small amount in a narrow spot, how thin the mix is governs how far it reaches. You see which product is opened right in front of you, and the dissolving itself is visible through the glass wall from the waiting area.

If the cause is different, we don't recommend the treatment. If hollowing beside the mouth is what makes it look droopy, this treatment won't deliver the change you want. We say so, and point you in another direction. You're welcome to come in just for a consultation.

FAQ

Q1. Who performs the treatment? A: PECHE physicians who all work to the same injection standards, built from over 90,000 documented treatments. Small muscles crowd the mouth corner, and even a slightly off placement changes the set of the mouth — so the depth and dose are set by these standards, not by feel.

Q2. How much will my mouth corners lift? A: This isn't a treatment where they visibly rise. The force dragging them down eases and the droopy impression softens — and the size of that change depends on how strongly the muscle was pulling to begin with. We confirm the expected range together before starting.

Q3. Will my smile or speech turn awkward? A: The muscles that move your lips sit right beside this one, so a large dose or a wide spread could do that here. That's why we place only the minimum in a narrow spot, check your smile and speech together at 2 weeks, and top up if needed.

Q4. How often should I have it? A: Over 3–6 months, the downward pull gradually returns and the mouth settles back to how it was. Stopping doesn't leave you droopier than before you started. To keep the result, 3–4 months is the baseline interval — we recheck the set of your mouth then, and set the dose.

Q5. What should I look for when choosing a clinic? A: Whether they first separate muscle from hollowing as the cause, and whether they look at each side separately. Skip that confirmation, and you can end up treated when the cause was different — or with the height difference left in place.
